Job Overview:
A respected domiciliary care provider is seeking a full-time Care Coordinator to join their growing team. This is a key role responsible for ensuring that all care visits are scheduled efficiently and consistently covered, supporting vulnerable individuals to enjoy full and active lives in their homes and communities. Key Responsibilities:
Coordinate the permanent allocation of Care & Support Workers to customer visits
Maintain effective communication with care staff, customers, and the management team
Schedule new customer referrals and ensure intelligent route planning for staff
Liaise with the recruitment team to manage staffing levels and minimise service gaps
Respond to changes in customer needs and update care plans accordingly
Ensure all documentation and electronic records for both staff and customers are current
Collaborate with Field Supervisors on staff illness and holiday coverage
Ensure compliance with all office systems, processes, and reporting protocols
Prepare management reports and provide updates to senior leadership as required
Monitor customer wellbeing in collaboration with social workers and care professionals
Act as a line manager to carers, supporting their development and performance
Work closely with the wider care and admin team to deliver excellent service
Candidate Requirements:
Minimum NVQ Level 2 in Health and Social Care or 1 year of office-based experience in a similar role
Strong organisational and communication skills
A collaborative team player with a passion for delivering high standards of care
Comfortable working in a fast-paced and evolving environment
